Much of the personal information that we have comes directly from you.  You disclosed most of this on your insurance application.  We may have contacted you by telephone or mail for additional information.  We also keep information about claims history and the products and services you purchase from us.  We may also collect personal information from outside sources, including consumer reporting agencies.  This information may include motor vehicle records, credit reports, and medical information.  Depending on the nature of your transactions with us, you may be asked to provide us, or the insurance companies that we represent, or other entities working on behalf with information.  That information may include, for example, your name, address, birth date, phone number, health information, or social security number.

Our policy regarding dispute resolution:

Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgement upon the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.

Reservation of right to disclose information in unforeseen circumstances:

In connection with the potential sale of transfer of interests, this agency and its affiliates reserve the right to sell or transfer your information (including but not limited to your name, address, age, sex, zip code, state and county of residency and other information that you provide through other communications) to a third party entity that (1) concentrates its business in a similar practice or service; (2) agrees to be this agency’s successor in interest with regard to the maintenance and protection of the information collected; and (3) agrees to the obligations of this privacy statement.
